Оn the role of pro-fibrous cytokines in the pathogenesis of localized scleroderma

Abstract

The content and skin distribution of the transforming growth factor β 1 (TGF-β 1), transforming growth factor-β receptor type I receptor (TGF-βRI) and platelet-derived growth factor receptor a (PDGFR-α), which are key fibrosis mediators, were examined in ten patients with localized scleroderma and ten healthy volunteers by using the immunohistochemistry method. A reduced derma concentration of TGF-β 1+ cells (р = 0.007) and increased amount of TGF-βRI+ (р = 0.001) and PDGFR-α+ (р < 0.001) cells was discovered in the patients vs. the control group. Te reduced amount of TGF-β 1 in the affected loci can be apparently explained by its reduced production by cells taking part in the immune inflammation. Increased expression of TGF-βRI receptors (р = 0.001) and PDGFR-α in the foci of localized scleroderma confirms their important role in the pathogenesis of the disease and activation of the fibrosis process in the skin.

Role of gonococcal infection and its forms associated with other sexually transmitted infections in the development of clinical and morphological changes in the cervix

Abstract

Studied the clinical and morphological features of the mucous membrane of the cervix during gonococcal infection and its association with other sexually transmitted mainly through sexual contact. Established clinical features of inflammatory changes in cervical mucus and the peculiarities of its structure and state of repair processes in it for women with gonococcal and in association with her chlamydia and trichomoniasis infections. Revealed that the destructive changes in cervical mucus, and decreased reparative features a single-layered columnar epithelium are most pronounced in women with combined forms of gonococcal infections: gonococcal-chlamydial and gonococcal-trichomoniasis.

Efficiency and relevance of the use of an oxygen-ozone mixture in the complex treatment of acne in men

Abstract

The purpose of the study was to substantiate the relevance of the use of an oxygen-ozone mixture in the complex treatment of acne in men. Materials and methods: The study involved 46 male patients aged 15 to 27 suffering from acne vulgaris of moderate severity. patients from the control group received a standard therapy (systemic antibacterial drugs, benzoyl peroxide and adapalene as external drugs), and patients from the main group received an oxygen-ozone mixture in addition to standard drugs. Clinical evaluation of the intensity of the process was carried out prior to and after the treatment (using the Cook scale), and severity of scarring was assessed by the Vancouver scale; immune status parameters were also assessed. Results: positive clinical dynamics was observed in both groups. normalization of immune status parameters was additionally observed in patients from the main group. Conclusion: the study demonstrated the efficiency of a combined therapy including the introduction of an oxygen-ozone mixture in addition to traditional drugs, so this regimen of treatment can be recommended as a therapy for acne of moderate severity.

The article presents an analysis of clinical and medical history data of female patients suffering from inflammatory diseases of the lower urogenital tract caused by opportunistic microorganisms. The authors analyzed the results of the bacterioscopic and bacteriological examinations obtained from the patients. They present data on clinically significant associations of microorganisms. The authors studied the dependence of clinical characteristics of the course of the inflammatory process on the nature of associations of inflammatory agents, and presented the results of their study.

Assessment of the clinical significance of the chemiluminescence immuncassay in the diagnostics of the syphilitic infection

Abstract

The authors present the results of an assessment of the clinical efficacy of the use of the chemiluminescence immunоassay (CLIA) for determination of anti- T. pallidum antibodies in the diagnostics of the syphilitic infection with the use of IMMULITE® 2000 Syphilis Screen, a reagent kit developed on the basis of Tp17 recombinant antigen. The use of 306 samples of blood serum collected from patients suffering from different clinical forms of the syphilitic infection, 61 samples collected from active blood donors and 20 samples collected from persons with biologically false-positive immunochemical (serological) reactions for syphilis demonstrated a high clinical sensitivity (99.68%), specificity (96.30%) and reproducibility (100%) of the chemiluminescence immunоassay results. The high clinical efficacy of the study results combined with the speed of its performance and full automation of the laboratory study process are sufficient to recommend including the chemiluminescence immunоassay in regulated methods for primary examination (screening) of the population for the syphilitic infection along with treponemal tests such as immunoenzyme assay (IEA) and passive hemagglutination reaction.

Role of MRSA strains in the pathogenesis of severe atopic dermatitis in childhood. The ways of remission achievement

Abstract

This article presents results of examination of 63 patients with atopic dermatitis (AD) aged 3 to 18 years. It was demonstrated, that in 87.3% of cases their skin was contaminated by Staphylococcus aureus. In 58.2% of cases pathogenic strains of methicillin-resistant Staphylococcus aureus (MRSA) were detected. Direct correlation between the frequency of MRSA detection and severity of the disease (r = 0,68) as well as age of children (r = 0,58) was revealed. Authors revealed high frequency of multidrug-resistant strains to a variety of antibiotics, except mupirocin, vancomycin, clindamycin, and ciprofloxacin (in the total group of MRSA the prevalence of resistant strains to these antibiotics ranged from 2% in mupirocin to 18% in ciprofloxacin, depending on the severity of AD). Algorithm of the effective combined therapy with mupirocin containing topical drugs (Supirocin ointment, Supirocin B ointment) is presented. This algorithm allowed to achieve stable clinical remission in 100% of cases.

Experience of effective treatment for patients with severe psoriasis

Abstract

In this study was summarized the clinical monitoring of 6 patients with a vulgar severe psoriasis receiving a short course of Ustekinumab — antagonist of IL 12/23. Clinical monitoring demonstrated high efficiency of treatment with achievement of clinical remission and appreciable improvement in all patients; conservation of remission of psoriasis process till 6 months with regress of PASI more than 75 % from initial point in 5 from 6 patients; improvement of quality of life, absence of complications and adverse events during the treatment and the subsequent observation.

Current approaches to the external therapy of psoriasis

Abstract

Topical glucocorticosteroids rank first among drugs for the external therapy of psoriasis. When selecting a drug, it is necessary to take into account the disease sensitivity to different classes of topical steroids. Dermovate (clobetasol propionate) is the most efficient drug for the external therapy of moderate to severe psoriasis. When this drug was used as a part of the complex treatment of psoriasis, regression of eruptions was observed within three weeks in 97.4% of patients, and in 91.5% of patients if the process was localized in the area of palms and soles. Major indications for its use are torpidity with regard to the previous therapy, localization in the field of palms and soles, and substantial reduction in the life quality.

Current drugs acting on Toll-like receptors in the treatment of inflammatory dermatoses M.A. iONESCU

Abstract

The article presents the results of a study of the oil-in-water emulsion comprising TLR2-regul®, a complex patented by Uriage Laboratories. At the first stage of the study, skin explants from healthy donors were incubated in the physiological salt solution, oil-in-water indifferent cream excipient, with monoclonal anti-TLR2 antibodies and with the oil-in-water study emulsion, and then samples in each groups were divided into three sub-groups and incubated with extracts of M. furfur, p. acnes и S. aureus. The immune-enzyme assay method demonstrated a significant reduction in the production of IL-8 and higher quantity of β-defensin 2 in case of incubation with the study emulsion vs. the control group. At the second stage, the authors assessed the efficiency of the study emulsion in the treatment of patients suffering from seborrheic dermatitis (n = 143), acne (n = 34) and atopic dermatitis (n = 48) as a monotherapy vs. the cream base. After four weeks, patients with seborrheic dermatitis had a considerable reduction of erythema, peeling, itching and stinging, reduction in the number of recurrences; after three months, the group of acne patients had a considerably reduced number of inflammatory elements; after three weeks of treatment, patients with atopic dermatitis had a considerable reduction of erythema, infiltration and exudation as well as itching and shrinking. Thus, the study drug suggests new approaches to the treatment of inflammatory skin diseases with the pathogenesis complicated by P. acne, S. aureus or M. furfur due to its ability to modulate the immune response.
