Современные подходы к лечению андрогенетической алопеции

Аннотация

Большинство случаев выпадения волос обусловлено андрогенетической алопецией (АГА). Для европеоидной расы вероятность появления этого заболевания достигает 80% для мужчин и 42% для женщин. Золотой стандарт лечения АГА включает миноксидил и/или финастерид. Эффективность этих препаратов умеренная, рост волос наблюдается далеко не у всех пациентов. Более того, оба средства имеют значимые побочные эффекты. Для лечения АГА у женщин может использоваться только миноксидил. В работе анализируются современные представления о молекулярных механизмах патогенеза и методах лечения АГА с их ограничениями и недостатками. Освещены перспективные подходы к лечению АГА, способные превзойти по эффективности и безопасности миноксидил и финастерид, их сегодняшнее состояние и перспективы. Обсуждаются средства на основе факторов роста и цитокинов, ингибиторы 5а-редуктазы местного нанесения, антагонисты андрогеновых рецепторов, активаторы Wnt-сигнального пути, аналоги и антагонисты простагландинов, а также процедура плазмотерапии обогащенной тромбоцитами плазмой.
